Symptoms

On 11.5.10 in Production:


When attempting to access the Benefits Service Center form,the following error occurs:

APP-FND-01388 Cannot red value for profile option FND_DEVELOPER_MODE in routine &ROUTINE

ORA-01403 no data found.
FRM-40735 WHEN-NEW-FORM-INSTANCE trigger raised unhandled exception ORA-06508



Steps To Reproduce:
The issue can be reproduced at will with the following steps:

1. Go to US Super HRMS Manager Responsibility
2. Benefits Service Center
3. Select date track option and receive the APP-FND-01388 error. The form thus hangs
