R12:Unable to Create Operating Unit From HR or Accounting Setup Manager (ASM) (Doc ID 428828.1)

Symptoms

This issue occurs both in HR and the Accounting Setup Manager.
  
Scenario 1: On the Organization form, when attempting to create an Operating Unit, the legal entity LOV does not have any values. 
 
Scenario 2: In Accounting Setup Manager, when attempting to create an Operating Unit,  an error is received:

java.sql.SQLException: ORA-20001 The type of organization is invalid. ORA-06512: at  "APPS.HR_ORGANIZATION_API", line 5857 ORA-06512: at line 1

The following query returns no rows:
SELECT
ENABLED_FLAG ,START_DATE_ACTIVE, END_DATE_ACTIVE
FROM HR_LOOKUPS
WHERE LOOKUP_TYPE = 'ORG_TYPE' AND LOOKUP_CODE in ('OU','BG' );
