User Creation Fails in Oracle E-Business Suite if the User all ready exists in Oracle Internet Directory

Symptoms

When attempting to create a new user on User Define Form after Single Sign-on Integration with Oracle Ebusiness Suite Release 11i or Release 12, the following error occurs.

SAMPLE ERROR
ORA-20001 Unable to call fnd_ldap_wrapper.create_user due to the following reason:
A user with the given username already exists. Please use a different
username. (USER_NAME=SUPPDUPE@DOMAIN.COM)""

STEPS
The issue can be reproduced at will with the following steps:

1. Integrate Oracle Ebusiness Suite with Oracle Application Server 10G components like Oracle Single Sign-on, Oracle Internet Directory using either of the following documentation :

2. Login to Ebusiness Suite instance as SYSADMIN user and navigate to User Define Form

3. Create a new FND USER on the User Define Form and the error reproduces
