Symptoms

Attempting to upload a batch accounted transactions leads to an error "Exception Name: oracle.apps.bne.exception.BneFatalException - Program error Log File Bookmark: 206198".

After this error, no one can log into the system at all. When users try to log in they get the following error:
"Oracle error &ERRNO: &REASON has been detected in &ROUTINE."
OR
"SQL_PLSQL_ERROR"

From the BNE log the following error is shown:

PM ERROR BneBaseSQL.executeQuery: Exception while running query. Error Code: 1000,       Message: ORA-01000: maximum open cursors exceeded 
PM ERROR BneBaseSQL.executeQuery: Query: SELECT TASK_NUMBER
TASK_NUMBER,TASK_NAME TASK_NAME FROM PA_TASKS_EXPEND_V WHERE PROJECT_NUMBER = :1 AND TASK_NUMBER = '1309' ORDER BY TASK_NUMBER ASC 
PM ERROR BneBaseSQL.executeQuery: Args : EGPKE-Con-BO 
PM ERROR BneBaseSQL.executeQuery: Stack trace: java.sql.SQLException:
ORA-01000: maximum open cursors exceeded
