Workflow Has No Performer Error using CUGTASKS

Symptoms
When trying to use "Email Notification Workflow for CIC task" associated to a task type to launch a notification manually from a task using the "Launch Workflow" button, you get the message:
“Workflow notifications are send successfully”.
But, when check in Status Monitor, the workflow finish with error and the error message is:
“Error in Workflow CUGTASKS/XXXX-YYYY 3120: Activity 'CUGTASKS/XXXX' has no performer.”
Task Type setup:
Item Type = Email Notification Workflow for CIC task
Workflow process = CUG_EMAIL_NOTIFICATION
Rule = Dispatch
Notifications and Schedulable have the boxes checked.
Steps to reproduce the problem:
1. Create a Service Request
2. in the Task tab create a Task using a Task Type that has the "Email Notification Workflow for CIC task" workflow associated to it.
3. Save the Task
4. click the "Launch Workflow" button
5. you get message:
“Workflow notifications are send successfully”.
6. as System Administrator navigate to Workflow Administrator > Status Monitor
7. query for Internal Name = JTFTASK and Item Key = %task-number%
8. the wrokflow has status error and the error is:
“Error in Workflow CUGTASKS/XXXX-YYYY 3120: Activity 'CUGTASKS/XXXX' has no performer.”
