On 12.0 in Production:

When attempting to create a new grade rate, the following error occurs.

ORA-01403: no data found
ORA-06512: at "APPS.HR_RATE_API", line 270
FRM-40735: ON-INSERT trigger raised unhandled exception ORA-01403

Steps to Reproduce:
The issue can be reproduced at will with the following steps:

1. Select a HRMS Manager Responsibility
2. Go to Workstructures > Grade > Grade Rate
3. Enter a grade rate and save
4. Error Occurs

Recent upgrade to Release 12.
