Not Able To Upload File Using The Attachments Form (Doc ID 453169.1)

On in Production:
When attempting to upload attachment to service request, the following error occurs.

 "The Page cannot be found"

The issue can be reproduced at will with the following steps:

(R)Customer Support
1) Search for a service request
2) Go to Details Tab
3) Click at button 'Attachments...'
4) Select Category = Miscellaneous and Data Type = File
5) Select the file to be uploaded and click at "OK" button
6) Fail to upload the file - Encountered error message "The Page cannot be found"
