Pickup and Delivery Location Details Are Missing When Re-Using an Order Template

-- Problem Statement:
After creating a new order base from an order base template, if the same template is re-used, the source and destination location details are blank.  The same is true for an order release.

Expect to have the pickup location and delivery location fields populated when re-using a template to create orders.

-- Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1. Create order base using order base template (Note that the template has already the predefined source
and destination location).
2. After successful creation of order, click on "Create New Order" to create another one and select the template again.
3. Observe the template screen - the source and destination locations details are empty.
