ORA-01403 Error While Closing The Requisition Form Opened From The Notification (Doc ID 464784.1)

FORM:POXRQERQ.FMB - Enter Requisitions GUI

Symptoms

When attempting to close requisition form that has been opened from the notification summary,
the following error occurs:

ERROR
ORA-01403: no data found
Clicking Details button shows :
FRM-40735: Key-EXIT trigger raised unhandled exception ORA-06502
Clicking OK shows :
An Unexpected Error -6502 has occurred. An alert has been sent to the system administrator

The following error can also sometimes appear :

FRM-41085: Error getting Group Row Count

Steps To Reproduce:

1- Create a Requisition and submit it for Approval
2- The approver opens the Notification for the Requisition.
3- In the Notification the Approver clicks on 'Open Document' Link and then the Requisition form
Opens.
4- The approver reviews the information in the Purchase Requisition and then tries to close the
form
5- The approver sees the error messages
