This problem can occur on any platform.
When attempting to upgrade E-Business Suite Release 11i instance to Release 12,
the following error occurs in the last stage of RapidWiz session.
Window Title: "Instal Oracle Applications - Post Install Checks"

RW-50016: Error: - database ORACLE_HOME environment file was not created:
File = /<s_base>/tech_st/10.2.0/<s_dbSid>_<host>.env
RW-50016 error: Error - TNSNames SQL*Net file was not created
File: /<s_base>/db/tech_st/10.2.0/network/admin/<s_dbSid>_<host>/tnsnames.ora APPL_TOP

The issue can be reproduced at will with the following steps:
1. Following Oracle Applications Upgrade Guide: Release
11i to Release 12 Release 12 Part No. B31566-01.
Performing the step number 3 of chapter 2. - Step 3 Run Rapid Install (required)
On the Wizard Operation screen, select "Upgrade to Oracle Applications Release 12".
On the Select Upgrade Action screen, select "Create Upgrade File System".
In the associated screen flow, enter the parameters required to set up your new environment. Then,
run Rapid Install.
