R12:AP/XLA: Create Accounting Gives NULL Cash Clearing Account and 95311 and 95353 Errors for Cash Clearing

Symptoms
The Create accounting output (XLAACCPB) from Payables displays errors:
95311 The segment value for the account code combination specified in line 2 is not assigned to the ledger NPO. Please update the segment value assignments for the ledger in the Accounting Configuration page or use another segment value.
95353 Subledger Accounting was unable to derive an accounting code combination using the account derivation rule Cash Clearing Account owned by Oracle. Please review the account derivation rule and make sure it derives a valid accounting flexfield combination for the source values passed for the transaction.
The Cash Clearing CCID is not populated and the Liability CCID is populated in all cases.
Changes
Instance has recently had a new install or upgrade to R12.
Verified that this is not related to Bug 6338819 - R12 PAYABLES PAYMENT ACCOUNTING OPTIONS COLUMNS ARE NULL FOR NEW OUS
