Group of Lines Surcharge Modifier using Item Category and Item Amount is Not Applying Correctly (Doc ID 550002.1)

Symptoms

-- Problem Statement:
On 11.5.10 in Production:
Find group of lines modifier using item category and item amount is not applying correctly. It does not apply when ordered quantity > 1. It does apply if ordered quantity =1.

EXPECTED BEHAVIOR
Expect it to apply regardless of the ordered quantity as long as the total item amount is within the range specified in the modifier setup.

-- Steps To Reproduce:
The issue can be reproduced at will with the following steps:
Modifier created:
header:
type: surcharge list
automatic, active are both checked
modifiers summary:
level: group of lines
modifier type: surcharge
automatic and override are checked
pricing phase: Modifiers for BOOK Event
product attribute: item category
product attribute value: NEW.MISC
volume type: item amount
break type: Point
between 0 and 299.99
application method: lumpsum
value: 45

Added items to price list:
1 item1 $10
2 item2 $16
3 item3 $15
--All these items are assigned to the item category used in modifier setup.

Created sales order
1 item1 quantity=10
2 item2 quantity= 5
3 item3 quantity=5
--Modifier is not applied

Created sales order
1. item1 quantity =1
2. item2 quantity =1
3. item3 quantity =1
--Modifier is applied

Requeried first sales order
Changed all lines quantity ordered to 1 and saved. (entered change reason since order is booked)
--Now modifier applies.

The issue has the following business impact:
Due to this issue, users cannot correctly apply this modifier.
