R12 ARXCCU Credit Limit Usages Report Fails to Submit With 'APP-FND-01564: Oracle error 1406 in FDFGVD' (Doc ID 550240.1)

-- Problem Statement:
On 12.0.3 in Production:
When attempting to run the Credit Limit Usages Report,
the following error occurs while submitting the report

APP-FND-01564: Oracle error 1406 in FDFGVD

Cause: FDFGFD failed due to ORA-1406: fetched column value was truncated

The SQL statement executed at the time of the error was
select organization_id, name value, name description, vnl('N','N'), nvl(to_number(null),-1) ...
from hr_operation_units
where 1=1 and name=:X

The issue can be reproduced at will with the following steps:
1. Navigate to the request submission screen
2. Try to run the Credit Limit Usages Report

The issue has the following business impact:
Due to this issue, users cannot run this report


Fresh Install Release 12 or upgrade from any prior release
