External Learning Automaticlly Approves and No Notification is Sent To Approver

On 11.5.10 in Production:

External learning enrollments are automatically approved and not sent to approver.
Our business rule states all  external training must be approved by approver before being attended.

On customer workflow, HR_APPROVAL_REQ_FLAG for External Learning Entry is YES
but there is no HR_APPROVAL_REQ_FLAG for review page.

Responsibility "Learning and development"
External Learning / add / Fill the field and click on button Apply.
There is no page to review and to send notification to an approver

The issue has the following business impact:
Learners can attend any external training without prior approval thereby presenting unexpected training expenses to our organization.
