Errors: ORA-01422 ORA-06512 When Approving Service Contract (Doc ID 559050.1)

Symptoms

When attempting to approve a contract by a REASSIGNED user ,after the timeout has occured. the following error occurs:

ERROR:
ORA-01422: exact fetch returns more than requested number of rows ORA-06512: at
"APPS.WF_NOTIFICATION", line 4757 ORA-06512: at line 1

 Steps To Reproduce:
The issue can be reproduced at will with the following steps:

1) Create a contract
2) Submit for approval
3) The contract workflow has a timeout for 5 days for the notifications
4) We reassigned the contract from one user A to B as A was on leave .
5) B did not approve for 7 days .
6) B Went to approve on the 8th day and got the oracle error as :
ORA-01422: exact fetch returns more than requested number of rows ORA-06512: at
"APPS.WF_NOTIFICATION", line 4757 ORA-06512: at line 1
