FAGDA: Account Generator Fails to Generate the Revaluation Accounts REV_AMORT and REV_RSV

FAGDA
Symptoms
The FAGDA: Generate Accounts program fails with the following error message:
Account Generator failed to generate a new code combination:
Account Type: REV_RSV
Book: <book name>
Distribution ID: <distribution_id>
Concatenated Segments:
Cause: The code combination for the generated segment does not exist and cannot be created because it would contain invalid segment values or because it has been disabled.
Action: Please review the following message for a detailed explanation of why the account could not be generated.
Error: function FAFLEX_PKG_WF.START_PROCESS returned failure
The same error message also for the account Type REV_AMORT
The issue can be reproduced at will with the following steps
1. Fixed Assets Responsibility
2. Other > Request > Run > Generate Accounts
Generate Accounts can fail with the same error during depreciation run as well.
