Getting AR_ACH_FAILED error when running Automatic Remittance (Doc ID 564195.1)

Symptoms

Automatic Remittance Execution Report shows the following error for ACH Bank Account transactions:

APP-00001: Unable to find message: AR_ACH_FAILED

<Patch 6122705> has already been applied, but this did not fix the issue.

ibypregb.pls version 115.21

Remittance log shows the following:

Registering Bank Account Instrument...
