All Forms Based Windows Hang after a new Installation of eBusiness Suite R12

Unable to access forms based windows after a new Installation of eBusiness Suite R12.
Any attempts hang, and no error message is displayed in the browser ( Internet Explorer or Mozilla Firefox ).
All jsp screens are working with no issues, and the OC4J forms is up and running ( can be confirmed with adapcctl.sh status command ).
You can access the forms directly via http://hostname.domain:port/forms/frmservlet by setting s_appserverid_authentication to "OFF" in the context file.
