Custom Actions Disappeared After Patch Installation

This problem can occur on any platform.
-- Problem Statement:
On OTM 5.5.3:
After Installing OTM patches, the custom actions added to the 'original' screen set have disappeared.
-- Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1- Log on to OTM as the DBA
2 - Navigate to: Configuration and Administration > User Configuration > Screen Set Manager.
3 - Modify the original screen set and add custom actions.
4 - Install any OTM patch.
-- Business Impact:
The issue has the following business impact:
Due to this issue, users cannot find the added custom actions.
