Advanced Collections Search Screen - Multiple Errors in R12

Symptoms

When opening the IEX search screen, the following errors appear in the form and it is not possible
to search:

FRM-41045: Cannot find item: Invalid ID
FRM-40105: Unable to resolve reference to item IEXLSSIT_QUICK.RG_SEARCH
APP-IEX-240176: Customer By can not be null
