Nonconformance History Plan Has No Records (Doc ID 602541.1)

On 11.5.9 in Production:
Find you created your Nonconformance master plan and all the child plans using the NC
template. For each of your plans, Nonconformance Master, Nonconformance Detail
and Disposition Detail you have a history plan as a child. When you enter a
nonconformance into the NC Master plan it is supposed to create a child record
in the History plan. The history plan is supposed to capture any changes to
the parent. What you  are seeing is that when you save a NC Master record, no
history record is saved (or at least you cannot see it in the Inquiry form).

Expect ...history record to be saved

The issue can be reproduced at will with the following steps:
1. Parent record created in the 1M1_NCM_MASTER plan.
2. You can see that the 1M1_NCM_MASTER_HIST plan is a child of the 1M1_NCM_MASTER plan.
3. The 1M1_NCM_MASTER plan record is saved.
4. Checking the 1M1_NCM_MASTER_HIST plan, the only record in that plan is one I entered directly
on 12-Apr-06. No other records exist in the history plan although we have dozens of records in
the 1M1_NCM_MASTER plan.
