Requisition Approval Workflow Ending With 3146: Commit happened in activity/function Exception When Launching Createpo Workflow (Doc ID 603475.1)

Symptoms

When approving requisitions, the workflow fails when it launches the createpo workflow.

Error in :MAIN_REQAPPRV_PROCESS/LAUNCH_CREATE_PO_WF ERROR #EXCEPTION

Error Stack:
3146: Commit happened in activity/function 'MAIN_REQAPPRV_PROCESS:LAUNCH_CREATE_PO_WF/PO_REQAPPROVAL_LAUNCH.LAUNCH_CREATEPO_WF'

The issue happens intermittently when doing these steps
1. Approve requisition
2. The workflow fails when launching the createpo workflow. A Purchase Order (PO) is not automatically created for the requisition.
