Symptoms
Issue was seen both on a fresh release 12 install and post upgrade to 12.0.4.
Using the FNDRSRUN form (version 12.0.29.12000000.11), a "FRM-40735: POST-QUERY trigger raised unhandled exception ORA-01403" is seen when using the "View Details" button on a concurrent request belonging to any user other than the one currently logged in as (OPERATIONS and "System Administrator"). All other buttons on the form (Cancel, Hold, Diagnostics, View Log) work properly.
