When creating a contact on the Service Request form after entering a Serial Number, the following error appears when saving the record:


ORA-01400: Cannot insert NULL into ("CS".CS_HZ_SR_CONTACT_POINTS".CONTACT_TYPE") in Package CS_ServicesRequest_PVT Procedure Create_ServiceRequest

Steps To Reproduce:

(R) Service
(N) Service Requests > Create Service Requests

 - Enter Customer and Contact information
 - Enter a Serial Number.
 - Navigate to the Contacts / Addresses tab
 - Create a Contact.
 - Try to save the record

 => Error appears.
