Symptoms

In release 12.0.5, the flexfield structure name and each segment name is shown twice in the LOV on
the Key Flexfield Values form

--Expected behavior
The structure name and each segment should only be shown once.

Setup > Financials > Flexfields > Key > Values
Find flexfield by structure
