Attempting To Assign Ship Method To Org For Existing Freight Carrier Errors With: APP-WSH-231106-Unexpected Error Pos 20 Updating WSH_ORG_CARRIER_SERVICES (Doc ID 730693.1)

When trying to assign an existing freight carrier to an org, by clicking on a ship method and opening the Org Assignments window,
then selecting the organization (ie Michigan) and clicking save, the following error is raised:

APP-WSH-231106-Unexpected errors encountered at position 20 when updating
WSH_ORG_CARRIER_SERVICES

Details: Exception Message, EXCEPTION: No Data Found, Error User-Defined Exception, Code 1

 
The issue can be reproduced at will with the following steps:

1. Select Order Management Super User Responsibility
2. Navigate to: Shipping > Setup > Freight Carriers, Cost Types > Freight Carriers
3. Open an existing freight carrier
4. Click on a ship method
5. Click Org Assignments window
6. Choose Michigan in the organization assignments window and save. Get the error.

Changes

Upgraded to release 11.5.10 from a lower release of 11i. 
<Patch 4317851> has been applied to stop future occurrences of this issue suggested in <Note 334422.1>: APP-WSH-231106: Error When Attempting To Enable Org To Carrier Service
where issue is fixed in file WSHFXCSM.fmb and version 115.117.11510.9 or higher. 

In addition, error only occurs when trying to SAVE the record and only when assigning a freight carrier to a particular inventory organization (org_name = Michigan).
Issue does not happen when trying to assign the same freight carrier to other inventory organizations.
