R12: Http Server Crashing Frequently With Segmentation Fault (NAS or SAN filesystem) (Doc ID 732857.1)

Symptoms

E-Business Suite Applications Technology Stack, HTTP Server issues

When running R12 you notice that the http server is crashing frequently.

The http server log directory has the following shared memory crash files:
dms_metrics_XXXX.shm.mem
mod_oc4j.XXXX.shm.mem
mm.XXX.mem

The crashing of the http server also has some serious effects on the oacore and forms jvms, so expect to see some instability with the below errors:
