Blank Page Accessing R12 - Application.log shows "html: chain failed", "Unable to create user session" (Doc ID 734692.1)

Symptoms

When attempting to login via the typical http://server.domain.com:8000/, the next expected page is the local login page of http://server.domain.com:8000/OA_HTML/AppsLocalLogin.jsp.  Instead, a blank page is returned and users cannot login.

Using the standard debugging methods of <Note:422419.1>, the following is seen in the $INST_TOP/logs/ora/10.1.3/j2ee/oacore/oacore_default_group_1/application.log:

Changes

The current pattern from customer service requests is that the issue primarily effects cloned instances.  The original instance will work fine, but the clone will have the problem described here.
