Script poxukpol.sql Fails During R12 Upgrade

Symptoms

During upgrade from Release 11.5.10.2 to Release 12.0.4, the script poxukpol.sql fails to process successfully.

The AD worker log file shows the following error information:

To reproduce the issue, follow the steps to upgrade to Release 12.0.4.  This particular task is using the adpatch utility to apply the R12.0.4 upgrade database driver u6394500.drv.

The workaround is to skip the failed AD worker, using the adctrl utility - this allows the upgrade to continue processing.
