Symptoms
When attempting to enter a sales order and trying to select a custom header Descriptive Flexfield(DFF), the following error occurs:
ERROR
FRM-92101: There was a failure in the Forms Server during startup.
This could happen due to invalid configuration. Please look ino the web-server log file for details.

STEPS
The issue can be reproduced at will with the following steps:
1) Order Management Super User Responsibility
2) Order organizer > create a new order
3) Enter customer number and order type then click on custom DFF
4) Error message is raised
