Unable To Delete A Proposal: Attempt to access dead entity in FndLobsEO (Doc ID 743395.1)

Symptoms

On 12.0.3 in Production:
When attempting to delete proposal, the following error occurs:

ERROR
"You have encountered an unexpected error, Please contact the system administrator for assistance."

Exception Details:
oracle.apps.fnd.framework.OAException: oracle.jbo.DeadEntityAccessException:

JBO-27101: Attempt to access dead entity in FndLobsEO, key=oracle.jbo.Key[xxxxx] at
oracle.apps.fnd.framework.webui.OAPageErrorHandler.prepareException(OAPageErrorHandler.java:1247)
at oracle.apps.fnd.framework.webui.OAPageErrorHandler.processErrors(OAPageErrorHandler.java:1435)
at .....

The issue can be reproduced at will with the following steps:
1. Sales Dashboard > Proposals > Update Proposals page.
2. Now click on Delete button, and say Yes to Confirmation
Get error.
