Journal Approval Preparer does Not get a Notification (Doc ID 743674.1)

Steps to recreate the issue:

1. Preparer X creates a journal for $30,000 and submits for approval.
2. Approver Y who is X's direct supervisor approves journal but Y only has limit of $2000.
3. Request is then routed to Approver Z who is Y's direct supervisor with approval limit of $50,000.

Preparer X did not receive e-mail notification that journal was forwarded to next approver.

Behind the scenes it is found that:
The system has set the status of related notification of message_type 'VERIFY_AUTHORITY_FAILED' in wf_notifications table to 'CANCELED'.
As as result, the notification is created but e-mail is not sent.
