Symptoms

XML/BI Publisher 5.6.3/E Business Suite R12

Created a template using insert->cross table wizard.
When previewed in Microsoft Words add-in it is ok.
However, when run the following error is received:

Caused by: oracle.xdo.parser.v2.XPathException: Namespace prefix 'crosstab'
used but not declared. at
oracle.xdo.parser.v2.XSLProcessor.reportException(XSLProcessor.java:806) at
oracle.xdo.parser.v2.XSLProcessor.newXSLStylesheet(XSLProcessor.java:571)
