User is End Dated after Resetting Password via Login Assistance Link (Doc ID 749562.1)

Symptoms

On 12.0.4 in Production and Test:
A user resets the application password using the Login Assistance link at the login screen. The following error occurs after the password is reset and a workflow for the same user is approved there after, it failed when attempting to notify the user:

The following error occurs when selecting a self service function after resetting the user's password from the Forgot Password link on the log in page:

Error:
'The HR Person attached to your FND_USER does not have a valid Workflow role'.



Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1. User resets their password using the Forgot Your Password link at the login screen.
2. A workflow is then approved for the same user.

The issue has the following business impact:
Due to this issue, it's delaying the time it takes to process transactions.
