Symptoms

On R12.0.4 in Production:

Why after the upgrade to R12, only some users, during Custom Workflow approval for Budget,
are experiencing the following error.

      An Unexpected Error-2002 has occured. An alert has been
      sent to the system administrator.
      ORA-20002: 4016: User/Role relationship for user 'username=<USERNAME>'
      and role '~WF_ADHOC_name=<NAME>' already exists.
      ORA-06512: at "name=<NAME>", line 183


Customer creates a list of users and is utilizing API to upload them to create adhoc roles:

      wf_directory.createadhocrole
                  (role_users => l_role_users,
                   role_name => l_role_name,
                   role_display_name => l_role_display_name)
