Email Notification Fails with Status 'OPEN' 'FAILED' After Launching Workflow 'UMXLHELP' For 'Forgot Password' Under 'Login Assistance link'

On 12.0.6 in Production:
Find that User do not receive and email notification for resetting password after launching Forgot Password Function (UMXLHELP) under Login Assistance Link on Home page. When checking under the WF_NOTIFICATIONS table, the status is 'OPEN' 'FAILED'.
Expect to send email notification to the User.
The following continues to work:
Normal TEST MAILER works fine.
The issue can be reproduced at will with the following steps:
1. On the Login page, select the Login Assistance link.
2. Provide the User under section 'Forgot Password'
3. Notice that email notification never reaches the user and stays in OPEN FAILED under the
The issue has the following business impact:
Due to this issue,user cannot change or reset his/her own password.
