11i: Debrief Install Does Not Set The 'INSTALLED AT' Address Of The Instance (Doc ID 802258.1)

Symptoms

When creating a customer instance in the Install Base (IB) through Field Service Debrief, the instance is created with a blank 'Installed At' address.

EXPECTED BEHAVIOR
Expect the installed at address to be populated with the address from the Service Request that is associated with the debrief.

Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1. Create a Service Request.
2. Create a task, and assign it.
3. Debrief the task, and enter a material line that will create a customer product.
4. Complete the debrief.
5. Check the created instance in the Install Base.
