'To' Column in Workflow Notification Action History Shows 'Workflow System' Instead of Actual User (Doc ID 803785.1)

Symptoms

On 12.0.4 in Production: 
When an approval is done, the 'To' column shows as "Workflow System" in workflow notification action history region.

Excepted Behavior:
'To' column should show correct user name instead of "Workflow System".

Steps To Reproduce:
This problem occurs when second/next approver receives notification to approve. Approver sees recipient display name as "Workflow System" instead of correct user name.
