AME Error ORA-06512 APPS.APP_EXCEPTION When Manager Terminated

Symptoms
An expense report is submitted using AME. There are 3 approvers for the report. Approver1 receives
the approval notification and successfully approves. AME routes report to Approver2. Before Approver2 approves the notification, Approver1 is terminated in HRMS and Approver1's application account is end dated. Approver2 clicks on the approval notification for the report and receives the error:

Notes
(i) If end date is removed from Approver1's application account, then the error does not happen
and Approver2 can access and approve the notification.
-- Steps to Reproduce ###
1. Create an expense report.
2. The report should go to A, B and then to C.
3. After submitting the report, it went to A.
4. A approved the report
5. A was then terminated so A's user record was end dated which made A as inactive
6. The report now errors.
