Invalid Usernames Not Recorded in FND_UNSUCCESSFUL_LOGINS (Doc ID 834946.1)

Symptoms

Invalid logins are recorded in Apache logs, but not reflected in FND_UNSUCCESSFUL_LOGINS.

1.  Query count(1) of the FND_UNSUCCESSFUL_LOGINS table and make note of the 
number of rows returned.
2.  Attempt to login with an invalid username or as the guest user.
3.  Query count(1) of the FND_UNSUCCESSFUL_LOGINS table and observe that the
number of rows returned has not increased.
