R12: OPMN start Failed After 11i Upgrade or After Cloning R12 (Doc ID 836688.1)

Symptoms

Web Tier/ Forms Tier: Services can not be started

When attempting to start opmn or any opmn managed service it fails on startup

 

opmnctl: opmn start failed

adformsctl.sh version 120.12.12000000.3
adformsctl.sh: starting OPMN if it is not running

opmnctl: opmn start failed.

adformsctl.sh: OPMN is not running and OPMN start attempt failed
adformsctl.sh: Could not start OPMN managed FORMS OC4J instance
adformsctl.sh: exiting with status 2

The key here is the opmn.log which may show:

09/06/01 14:47:05 [ons-secure] Failed to open wallet
(file:$INST_TOP/certs/opmn) [default password]: 28759
09/06/01 14:47:13 [ons-secure] Failed to open wallet
(file:$INST_TOP/certs/opmn) [default password]: 28759
09/06/01 14:47:21 [ons-secure] Failed to open wallet

Changes

You may have upgraded 11i to R12 or simply cloned an existing R12 environment.
