Approver Was Unable To Access Open Document From Workflow Mailer Notification

On 12.0.4, when attempting to use the 'Open Document' functionality from the workflow mailer notification and re-logging into E-business suite, the following error occurs:
Steps To Reproduce:
1. User receives a workflow mailer to approve a requisition.
2. After the user clicks on the 'Open Document' hyperlink, it displays the log in page.
3. After logging into the E-business suite, the exception message is displayed.
