This problem can occur on any platform.
Symptoms
If a user tries to run a self service report which is called using a WEB_HTML_CALL of
OracleOASIS.RunReport from a responsibility which is duplicated then the parameter form does not appear and they receive 'Page Cannot Be Displayed'.
