Custom Quote Layout - Source/Destination Locations Disappear

Symptoms
-- Problem Statement:
On 6.0, On a Custom Quote Manager Layout, the Source and Destination Location fields on the Quote Options page disappear when moving to that tab
EXPECTED BEHAVIOR
Expect that the fields show up
-- Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1. Create a QUOTE layout
2. Create a Screen Set to use for New, Edit operations
3. Make a menu entry to use this Screen Set
4. Call this menu item and press on <New> button
5. Fill the Quote ID, go to <Involved Parties> page, go to <Quote Request> page
6. See the effect on the top of the <Quote Request> page
