Symptoms

 On 12.0.6 in Production:
When attempting to launch the Price/Availability Query the following error occurs:

ERROR
"FRM-40010: Cannot read form
<PATH>/apps/appl/ont/12.0.0/forms/US/OEXOEAVA.fmx

-- Steps To Reproduce:

  1. Using the "Dist Order Management SuperUser" responsibility
  2. Click "Price/Availability Query" Form
  3. Oracle launches, Form does not display, and receive error status message
