TeleService - Create Service Request: FRM-41081: Cannot Move Item: Invalid Position (Doc ID 878561.1)

Symptoms

When attempting to use the Create Service Request form an error occurs.  

Error:

FRM-41081: Cannot move item: invalid position

Steps:

(R) Service
(N) Service Requests > Create Service Request

   => Error shows on bottom of window.

-or-

(R) Depot Repair
(N) Repair Orders

- Query Service Request

  => Pop-window shows the error
