Notes Entered for Tasks not Available in Notes Tab in 12.1.1

This problem can occur on any platform.
Symptoms
After upgrading to 12.1.1 from 11.5.9. When a task and note is entered, the task note is not appearing on the NOTES tab.
Ran the same test in 12.1.1 the note cannot be seen.
1. Create a task and click on Notes. Enter a note and save.
2. Open the Notes tab, it is missing the note entered above.
Do the same steps on 11.5.10 and it works fine.
