Cannot Enable Contract Terms In Contract Type Setup

The Enable Contract Terms checkbox and the Layout Template list of values
do not appear when you open the Create Contract Type page.
The issue can be reproduced at will with the following steps:
1. Navigate to Contract Types / Create Contract Type using the Contract
Workbench Administrator responsibility.
- There is no list of values for the Layout Template.
- There is no checkbox entitled 'Enable Contract Terms'.
Due to this issue, users cannot setup contract types to use Contract Terms
in the Contracts Workbench (Repository).
