Oracle Mobile Field Service - Version 12.0.0 to 12.0.6 [Release 12]

Find the following Laptop 'display' issue in MFS Laptop:
1) Item LOV is not displayed in "Create Parts Order" screen
2) Item , Serial and Instance LOV is not displayed in "Create SR" screen. This is causing issues with SR creation and saving. We are unable to create SR in Mobile FS.
3) "Create Personal Tasks" throws error --> "JBO-25058"
4) Account Number LOV in "Create SR - Section" does not have LOV
Steps To Reproduce:
1. Try to search Account Number
2. Try to Create Parts Order
3. When creating SR, try to select Item
4) Click 'Create Personal Tasks' quick link
