Purchase Order Workflow Errors with ERROR_MESSAGE=3835: ORA-20002: 2018: Unable to generate the notification XML - ORA-01403 No Data Found - ORA-06512: at APPS.WF_NOTIFICATION (Doc ID 973355.1)

Symptoms

After upgrading to 12.0.6 the following error is seen when opening some Purchase Order notifications. This is not specific to one user. The error message appears regardless of whether or not the email notification or the worklist notification is used.  This presents on both new notifications created before the upgrade to 12.0.6, as well as after the upgrade to 12.0.6. 

ERROR:
ORA-04103: no data found
ORA-01403: no data found
ORA-06512: at "APPS.WF_NOTIFICATION"...
